5-2-1-Almost None
The initial focus of the SCHPC is to promote healthy eating and physical activity for children, youth and their families.
The SCHPC has adopted the 5-2-1-Almost None prescription for health promoted by Nemours Health and Prevention Services as its major campaign.
5-2-1-Almost None states that when people engage in the following behaviors, their overall health improves:
- Eat at least 5 servings of fruits and vegetables daily;
- Receive no more than 2 hours of screen time per day;
- Participate in at least 1 hour of physical activity per day;
- Drink Almost No sugary beverages
The SCHPC is a Nemours Health and Prevention Services initiative in collaboration with the citizens of Sussex County.

PATCH (Planned Approach to Community Health)
The second main initiative is to utilize PATCH, which is a process that involves and enables members of a community to identify, plan and evaluate health promotion and disease prevention.
The Goal of PATCH The goal of PATCH is to increase the capacity of the community to plan, implement and evaluate comprehensive communitybased health promotion programs targeted toward priority health problems
PATCH Objectives PATCH aims to reduce the prevalence of modifiable risk factors for the leading causes of preventable disease, death, disability and injury.
